  • If you are a potential Registry applicant, you have likely arrived at this web site after:
     
    1. Seeing an advertised notice in an industry publication, calling for therapists to apply for a listing in the the Registry of Sexual Offender Treatment Providers

    2. Receiving word of it's existence from an agency/organization the applicant is working with

    3. Receiving an application invitation letter (for providers that have already expressed written interest), from the Interagency Council, via Click Here Publishing

    4. Finding the web site via word of mouth, search engine, link from a state affiliated web site, or other method.

  • All applicants MUST read the following pages.  These pages will assist you in the determination of the most appropriate application type.  You should choose one of the three (3) application forms that is the "BEST FIT"; based upon the type of treatment they intend to provide and taking into account education, training, work experience and qualifications.:
     
    1. Definitions of Sexual Offender Treatment, Risk Assessment, Types of Treatment Providers, Policies, and general information

    2. Specific explanations, definitions, and criteria for the three (3) types of Registry membership classifications listed in the Sexual Offender Treatment Provider Registry
       

  • After determining the most appropriate application type, we encourage all applicants to download a "hard-copy" version (see "Printed Registration Form" section) of the application that best meets your qualifications (available in several areas throughout the web site) to use as a guide when filling out the online application on this web site.  The hard-copy application will give you the opportunity to review the application questions and collect any necessary information required to complete the online application.
     
    • NOTE:  You may notice a few minor differences in the hard-copy application vs. the online application - different wording of questions, added or deleted questions.  The online application is the most recent version of the application and you should refer to it on any discrepancy.

    Read the points below to see why it is important to be well prepared before filling out the online application:
     

    • ALL questions are MANDATORY and ALL FIELDS MUST contain an answer.  If you fail to answer a question, you will be notified of the missing information (when the Save button is clicked) and you will not be allowed to save the page and move to the next page until the missing data is entered
    • The application form pages appear sequentially.  Thus, you must fill out each page, fully, in order to move to the next page in the application
    • The applications (both Registered Clinical and Psychiatric versions) are lengthy.  By reviewing the hard-copy version of the application you will be able to determine how much time you need to set aside to fully complete the online application.  This is important because you are required to FULLY COMPLETE the application once you begin.  If you do not fully complete the application, it will be discarded, your information will be lost, and you WILL NOT be entered into the Registry.   Again, 'partially completed' applications WILL NOT be saved and you will be required to begin again
       
  • After you have completed the entire application process, filling out the appropriate application form completely, your informatoin will be entered into the Sexual Offender Treatment Registry database.  Your information will be immediately available to anyone accessing this web site by searching the "Locate A Treatment Provider in Your Area" section of this web site.  The online Registry database is searchable using several search criteria
     
    • Reminder:  Misrepresenting your educational, training, ethical standards and qualifications violates the professional ethics as established by the appropriate licensing board(s).  Such violations will be handled by the specific licensing board of the offending treatment provider.
       
  • Once you have carefully read all the above information, reviewed the aforementioned links, determined the most appropriate application type for you (matching the type of treatment you wish to provide and your qualifications), and downloaded/reviewed a 'hard copy' of the application for review, you can begin the application process by clicking the link below.
